Case Management

Case management is a collaborative ongoing process that seeks to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet a person’s health and wellbeing needs. It is characterized by advocacy, communication, resource management and promotes quality and cost-effective interventions and outcomes for each individual.

Based on the needs and values of each individual and in collaboration with all service providers the case manager links individuals with appropriate providers and resources. The case manager’s role is to ensuring that the care and support provided is safe, effective, client-centred, timely and efficient. This approach aims at achieving optimum value and desirable outcomes for all.
